Caregiving Assessment: Designing the Best Care Plan for Older Adults — Oct. 2nd

As the demands on caregivers continue to grow, social service professionals play a pivotal role in ensuring the well-being of both caregivers and care recipients when working with older adults and their families.

This training is designed to empower social workers and caregivers to assess, develop and customize care plans to support the unique needs and preferences of older adults in their care. Learn to assess the challenges, create a care plan to address the problems, and implement and/or monitor the care plan solutions as needed.

The program will include a review of the importance of safety measures at home via a safety audit, and implementing fall prevention strategies. We will also consider long-term care planning, initiating conversations with family members about future care preferences, exploring available care options, and creating a comprehensive care plan.  A Case Study will address the need for comprehensive caregiver assessments and equips professionals with the skills to provide tailored support to a very demanding family caregiver. The interactive training will encourage participants to share caregiving challenges in a supportive environment to facilitate peer learning as well as build skills and competencies in assessing the needs of care recipients.

About the Instructor:

Carmel Murphy-Kotyan, a member of our Network Trainers Hub, has worked in the Caregiving/Aging Industry for over 25 years, with Family Caregivers, Solo Agers and Retirees. She is a Gold Standard Caregiver Consultant, an End-of-Life Doula and a Grief Coach. As Director and Owner of CMK Home Care, LLC, a Quality-of-Life Caregiving Company, she offers nonmedical help and safety for elders in their place of residence, and training for their families and caregivers. She is the Founder of the Caregiver Support Project, designed to help guide families through the difficult journey of caregiving for their aging loved ones/spouse,

Objectives

By the end of the training, participants will be able to:

  • Understand and implement proper techniques for activities of daily living (ADLs) to ensure the safety and well-being of their care recipients in a safe home environment.
  • Demonstrate and learn to apply effective communication strategies in caregiving scenarios.
  • Create a comprehensive plan for family caregivers that considers the physical, emotional, and social needs of their care recipients.
