Effective Strategies for Home-based Assessment Certificate Program

Description

Our new Effective Strategies for Home-based Assessment Certificate is designed to simulate home environments and prepare health and human services professionals to conduct home visits and assessments for people with disabilities and older adults. This online self-paced certificate program is ideal for a wide range of social and health service professionals, including social workers, case managers, physical therapists, occupational therapy practitioners, registered dietitians, speech language pathologists, nurses, physicians, and physician assistants who support their clients at home. 

The training is organized into six modules with engaging activities to build skills in these critical content areas:

  • Importance of Assessment within the Home Environment
  • Team-Based Care in the Home Environment
  • Communicating with the Person and Family
  • Cultural Considerations for Conducting Assessments in the Home
  • Reliable and Valid Assessment Tools, Including Documentation
  • Ethical and Legal Practices within Home Assessments

This program was designed in collaboration with faculty and staff from Boston University School of Social Work and Sargent College of Health and Rehabilitation Sciences to demonstrate why home assessment is important, communication skills needed when conducting an assessment, and important considerations for conducting visits inside someone’s home.

This course has been funded by the Shipley Center at Boston University.

Contributing Authors:

Jordana Muroff, PhD, MA, MSW, LICSW, is a Professor and Chair of the Clinical Practice Department at Boston University School of Social Work.

Bronwyn Keefe, PhD, MSW, MPH is a Research Associate Professor, the Assistant Dean of Workforce and Professional Development at Boston University School of Social Work where she is also the Director for The Center for Aging and Disability Education and Research (CADER) and The Network for Professional Education.

Karen Jacobs, OT, EdD, OTR, CPE, FAOTA, FHFES, FIEA, is the Associate Dean for Digital Learning & Innovation as well as the Program Director for Online Post-Professional Doctor of Occupational Therapy Program and a clinical professor at Boston University Sargent College.

Craig Slater, PhD, MPH, is a Clinical Associate Professor, and the Director of the Interprofessional Education and Practice at Boston University Sargent College.

Amelia Paini, MSW, is our Curriculum Development Manager for The Center for Aging and Disability Education and Research (CADER) at Boston University School of Social Work. 

 

Objectives

  • Assess the personal, functional, social, and environmental factors related to collaborative home-based assessment
  • Explain how the assessment process supports choice, self-determination, and participation
  • Identify the legal and ethical considerations during a home-based assessment.
  • Apply cultural humility in supporting a diverse group of consumers
  • Apply home-based assessment skills using case scenarios, video, and interactive AI-simulations interactive simulations.
  • Identify strategies to support interprofessional collaboration in team-based care
  • Demonstrate competence and confidence in conducting home-based assessment

Course Outline

The self-paced online program is designed to take 5 hours to complete, and is organized into 6 modules: 

  • Module 1: Importance of Assessment within the Home Environment 
  • Module 2: Team-Based Care in the Home Environment 
  • Module 3: Communicating with the Person and Family 
  • Module 4: Cultural Considerations for Conducting Assessments in the Home
