Description

This online, self-paced certificate provides learners with the information needed to develop a person-centered plan by reviewing the key steps in an assessment of functional needs, as well as the skills to gather pertinent information, review available resources, and ongoing support through care transitions.

The six-course Person-Centered Case Management (PCCM) certificate, with 24 CEs, is an alternative to our five-course Case Management Certificate, with 20 CEs. It differs in that PCCM offers an additional course focused solely on person-centered planning to further this area of skill development.
